Song Structure:

The song begins with the Intro Riff.  The Intro Riff also crops up during the chorus.
During the verse the Verse Riff is played.  Essentially it's just an arpeggio over the chord changes.
The solo is played over the same chords as the verse.  Note that [12] means play the bent note that you've obtained from the 10th fret.  
During the chorus play Em7 as <022033> and C/G as <332010>.
The song ends on a chord of Em.
